ADULT DAY

Adult Day provides meaningful day activities in safe, supervised places away from your home. You can use this service to do things in your community. Harmony Life Group staff will help you become more independent with health and social needs. HLG staff will help you with activities of daily living, health maintenance, and supervision. This service cannot be used for employment or volunteering.


ASSISTIVE TECHNOLOGY

Assistive Technology includes devices, equipment, and appliances to help individuals do things on their own, so they need less help from others.


BEHAVIORAL IN-HOME HABILITATION

Behavioral In-Home Habilitation is a short-term service for when individuals are having a severe mental health event or behavior that keeps one from their normal daily activity. This service is provided in individuals’ homes. HLG staff will you with activities of daily living, health maintenance, and supervision. This is a habilitative service.


CHILD DAY HABILITATION

Child Day Habilitation takes place in a community setting. It provides teaching and staff support to meet the age- appropriate needs of your child, due to a disability or special health condition. The focus will be teaching and providing support to promote independence and personal growth while encouraging community integration. This is a habilitative service.


COMMUNITY INTEGRATION

Community Integration teaches self-help, appropriate behaviors, socialization, and adaptive skills. This service mostly takes place in the community. Individuals decide where and how often they want to go places. This service increases independence and personal choice, and interactions with people in the community. Harmony Life Group staff will assist with activities of daily living, health maintenance, and supervision. This is a habilitative service.


CONSULTATIVE ASSESSMENT

Consultative Assessment helps individuals stay in your current services while keeping them, and others, safe. HLG provider will observe individuals in their home and where day services take place, then does a functional behavioral assessment, and give recommendations. Behavioral interventions are developed, used, evaluated, and revised as needed. This is a habilitative service.


DAY SUPPORT

Day Support provides activities in a provider-controlled day location. HLG staff will encourage self-help skills, appropriate behaviors, socialization, and adaptive skills. This service is available when an individual does not have an employment goal. Harmony Life Group staff will provide help with activities of daily living, health maintenance, and supervision. This is a habilitative service.


ENVIRONMENTAL MODIFICATION ASSESSMENT

Environmental Modification Assessment is an evaluation to identify devices one may need or changes to be made to one’s home or vehicle. It looks at health and safety needs, how an individual accesses his/her your community and their independence. This service may be required before using Assistive Technology, Home Modifications, or Vehicle Modifications.


HOME MODIFICATION

Home Modifications are changes to individuals’ homes to make it easier and safer to get around and do things independently. Modifications provide better home access and the need for less help from other people.


HOMEMAKER

Harmony Life Group Homemakers help with general household tasks, such as cooking, laundry, errands, and cleaning. This short-term service can be used when the individual who usually does household tasks is not able to help. Homemaker services do not include care or supervision. This service is available to people under the age of 18, living in their family home. Other DHHS services can meet this need for people ages 18 and older.


INDEPENDENT LIVING

Independent Living is provided to individuals who reside in their own home. Our staff will teach skills for living independently and doing things in the community. We will help with activities of daily living, health maintenance, and social and leisure skills. This is a habilitative service.


MEDICAL IN-HOME HABILITATION

Medical In-Home Habilitation is a short-term service for when an individual is having a severe medical event that keeps them from their normal daily activities. This service is provided in home. HLG staff will help with activities of daily living, health maintenance, and supervision. This is a habilitative service.


PREVOCATIONAL

This services teaches general job-related skills needed to get a job in the community and can be used for up to 12 months. Harmony Life Group staff will assist with activities of daily living, health maintenance, and supervision. This is a habilitative service.


RESIDENTIAL HABILITATION – CONTINUOUS HOME

Continuous Home is provided in a provider-controlled home with shift staff. HLG staff encourage and teach skills for living independently and doing things in the community. We help with activities of daily living, health maintenance, social and leisure skills, and supervision. This is a habilitative service.


RESIDENTIAL HABILITATION – HOST HOME

Host Home is provided in the home of a HLG employee/supervisor. Individuals live with the employee and share daily life with the family in the home and community. HLG provider teaches you skills for living independently and doing things in the community. They help with activities of daily living, health maintenance, social and leisure skills, and supervision. This is a habilitative service.


RESIDENTIAL HABILITATION – SHARED LIVING

Shared Living is provided in the home of an HLG contractor. An individual will live with the contractor and share daily life with the family in the home and community. HLG contractor teaches skills for living independently and doing things in the community. Provider will help with activities of daily living, health maintenance, social and leisure skills, and supervision. This is a habilitative service.


RESPITE

Respite is a short-term service that can used when one cannot care for himself/herself. Respite is temporary relief to the usual caregiver. HLG respite provider will help with activities of daily living, health maintenance, and supervision. This service has yearly limits of 240 hours on the DDAD waiver and 360 on the CDD waiver.


SMALL GROUP VOCATIONAL SUPPORT

Small Group Vocational Support is provided at a business or in the community. Individual work alone or with a team to learn work skills and Behavior modification. The business pays HLG and HLG provider pays you. Individuals may interact with employees or customers. We will help you with activities of daily living, health maintenance, and supervision. This is a habilitative service.


SUPPORTED EMPLOYMENT – FOLLOW-ALONG

Supported Employment – Follow-Along helps individuals maintain their competitive employment. This service may be provided directly with individuals or by communicating with their employer for them. This is a habilitative service.


SUPPORTED EMPLOYMENT – INDIVIDUAL

This provides a job coach for one-on-one teaching to help individuals keep competitive jobs. This service is provided at an individual’s job. HLG will help with job skills and referrals for other resources. This is a habilitative service.


SUPPORTED FAMILY LIVING

Supported Family Living is provided in one’s family home. HLG staff will assist with teaching skills for living independently and do things in the community. HLG will help you with activities of daily living, health maintenance, social and leisure skills, and community access. This is a habilitative service.


THERAPEUTIC RESIDENTIAL HABILITATION

Therapeutic Residential Habilitation is an all-inclusive service. It is for when an individual has co-occurring disorders of developmental disabilities and severe mental illness. This service is provided by HLG staff in a agency residential setting and the community. This is a habilitative service.


TRANSPORTATION

Transportation provides rides to and from waiver services, separate from transportation during services. Transportation should not replace other options available to you, such as rides from friends or family. Transportation cannot be used for rides to medical appointments because other DHHS services can meet this need.
